How to Register for CYO Sports at St. Francis Xavier
Verify Eligibility
Ensure your child meets the CYO eligibility criteria outlined below.
Register Online
Visit the specific sport’s registration page and complete the online registration form.
Submit Required Documents
CYO Pre-Participation Form: Download, complete all sections, and have it signed by a physician. This form is valid for 13 months and must cover the entire season. Download here.
Birth Certificate: First-time participants must provide a copy, which will be kept on file.
Eligibility Criteria
Parish Membership
Parent(s)/guardian(s) are registered members of a Catholic parish within the Diocese of Cleveland or other approved areas.
School Enrollment
Your child attends a Catholic school within the Diocese of Cleveland or other approved area.
Parish School of Religion (PSR) or RCIA
Your child is enrolled in and regularly attends the PSR or RCIA in the current school year of the sponsoring parish and lives within the defined boundaries of the parish that would sponsor the team.

Required Forms
CYO Pre-Participation Form
Must include a physical exam, information and release section, and emergency medical authorization. All sections must be complete and current. The form becomes invalid if any information changes or if 13 months have elapsed since the date of the physical exam.
Birth Certificate
Required for first-time participants.
